Heart of the Valley Area Safety Training - Incident Investigation: Root
A systematic approach to incident investigation, identification of causal factors, and implementation of corrective actions is essential to managing an effective safety and health program. This seminar will help you determine the factors that cause preventable incidents and recommend effective corrective actions. Students will participate in mock accident exercises.
This class examines accident (incident) investigation and analysis as a means to improve safety performance, preventing injuries, property damage and financial losses. To have an effective safety program, you must identify and control hazards, and investigate accidents to determine the causes in order to prevent recurrence. Learn how in this session.
KEY TOPICS/GOALS/OBJECTIVES
Accident Investigation:
* Definition of an accident
* Direct and indirect costs of accidents
* Supervisor’s role in accident investigation
* Investigative process and procedures
* Deficiencies in the management system
* Why accidents may not get reported/human relation issues affecting accident reporting
Investigative Procedures:
* Securing the scene
* Utilize witness interviewing techniques
* Identifying the facts and analyzing the multiple causal factors
* Root cause analysis
* Data to include investigation reports
* Selecting and implementing corrective action
* Post action: hazard control measures and follow-up
