Most problems don't look like what they actually are.

A new system gets blamed for confusion that was already there before it arrived. Departments frustrated with each other are usually both right, and both missing what's happening on the other side. A push for "better data" often isn't about more information, it's about too many disconnected pieces that never added up to one clear picture in the first place.

The pattern repeats because the surface problem is usually easier to name than the real one. It's easier to say "we need a new system" than "we don't have shared processes." It's easier to say "we need more data" than "we don't trust what we already have."

None of this means the organization is broken. It means there's more happening beneath the surface than anyone's had the chance to see clearly, yet.
